Registration Requirements

We cannot process an application without the following: Copy of student’s Birth Certificate, Social Security Card, Last Report Card, Last Test Scores, Up-to-date Blue 680 and Yellow 3040 Forms (dated after June 1, 2013), Recommendation from Principal or Teacher from previous school and Our Lady Queen of Martyrs Entrance Examination.

Registration for Our Lady Queen of Martyrs Catholic School

Application Fee: $150.00 per student
Class Fee: $450.00 per student
Building Fund: $300.00 per family
Technology Fee: $100.00 per Student
Athletic Fee: $100.00 (per activity)
Grade 8 Fee: $300.00 due October 1, 2013. Includes mandatory 12 Week Preparatory Class for High School Entrance Exam and Graduation Fees.
Service Hours: 30 Hours per year or $20.00 per hour per family or $400.00 if paid in full by November 1st. Fifteen hours must be completed by November 15th and the remaining fifteen hours by May 1st. All families are required to commit 30 service hours each year of the school. This contribution helps keep the cost of tuition lower than actual student costs.
